EvNet Index to Questionnaires

The seventy-five (75) questionnaires made available in this section have been brought together from a wide range of sources. Some are well-designed, validated instruments; others are less sophisticated, but may be included for their creative content or because there is a lack of instruments to address that particular issue. Some have been created by EvNet researchers. Others have been generally available through listservs. Both are being made generally available on the Web. There are, however, some questionnaires which have copright restrictions. These are being made available only to EvNet members in a restricted area. Non-members who wish access must first become membes of EvNet. The questionnaires are being provided in a wide variety of formats - html, word, wordperfect, and pdf. They are categorized under five headings, as follows:

  1. Canadian Campus Computing Surveys: These eight surveys were constructed and put into the field by Campus Computing International (Canada) under the guidance of Carl Cuneo, Brian Campbell, Laila Bastedo, Edward O'Hara, and Selena Santi. The surveys collect institutional data on colleges and universities from the VPs of Information Technology, Heads of the Registrar and Library, Network Managers, Heads of Computing Support and Audio-Visual, Directors of Instructional Development Centres/Academic Technologies, and the Presidents of Faculty or Teachers' Associations.
  2. Disabilities, Education, and Computer Technology Questionnaires: These nine questionnaires are from the Adaptec Project at Dawson College in Montreal. Different versions of the same questionnaires (braile, tape, etc) are targetted at higher education students and service providers.
  3. Education Software Evaluation Questionnaires. These seven questionnaires are used as checklists in the evaluation of education software interface.
  4. Faculty/Teacher Questionnaires. These seven instruments measure teacher's experience and comfort level with using technologies, assess the resources available to teachers, their training in computer use, and their beliefs about the effectiveness of this teaching tool.
  5. Institution Questionnaires. These institution questionnaires assess the types and levels of institutional support given to teachers, or assesses the organizational costs and benefits of using technology in educational institutions.
  6. Learning Styles Questionnaires.These five questionnaires assess student's learning styles. These instruments provide a way to explore the possible link between education delivery modes and learning styles.
  7. Pre-Course Questionnaires. These four questionnaires in html, word, wordperfect, and pdf format can be used to evaluate or assess the abilities, needs, resources and attitudes of students before using certain technologies in education.
  8. Post-Course Questionnaires. These eleven mid to post-course questionnaires are aimed at outcome evaluation or comparative assessment (before-after).
  9. Software Utilities for Developing Online Evaluation Questionnaires. A small selection of software programs available on a shareware or commercial basis for developing online quizzes and tests in order to evaluate the performance of learners and trainees in courses. Also useful for administering online surveys.
